Dropped Connections / Disconnected Errors

If you often times get dropped from the game or receive disconnection errors can you please post them up here? Can you please let us know under what circumstances it happens, how long you were in the game, or idle, etc.

Please do not report any situations where you were disconnected after being idle for more than 20 minutes. Other than that, please elaborate so that we can work on finding a solution.
